Job Description Summary The Housing Case Manager is responsible for developing relationships with clients to support their independence and long-term housing stability and to assist them with integrating into their community. The Housing Case Manager also cultivates contacts with property managers and housing providers to promote positive, ongoing relationships. Essential Duties and Responsibilities Provide crisis intervention, long-term case management, advocacy, and aftercare/retention services to clients Develop effective and professional relationships with property owners/managers and housing providers Develop effective, trusting relationships with clients, with a focus on facilitating their independence and long term-housing stability Meet with client as often as necessary to provide ongoing support and guidance based upon individual client needs; update goal plan(s) as changes occur, and help to ensure client's retention of permanent housing Assess and provide life skill activities Assist client with integration into the community in which they live Meet with client in field as much as possible; at minimum, provide quarterly home visits Facilitate linkage to mental health, substance abuse and medical services, and provide transportation when needed Communicate with mental health, substance abuse, and medical provider to ensure continuity of care Assist clients in maintaining benefits and increasing benefits when needed Attend and participate in all case file, staff meetings, and required training Provide outreach to housing providers in a timely manner to effectively resolve landlord/tenant issues Organize documentation in client files and computer database, and complete all charting in a timely manner Participate in ongoing program evaluation, program development, implementation as requested Complete service statistics and reports as required by funders Communicate effectively with housing authorities on behalf of clients Qualifications Bachelor's degree with experience in the mental health field Demonstrated knowledge of case management Experience working with homeless individuals, preferably those living with mental illness and/or substance addictions Knowledge of housing and social service providers and programs, and of local and federal government benefits and entitlements Committed to improving the community through empowerment and dignity Able to work in a high tolerance Harm Reduction model with Members who have multiple barriers Able to evaluate data/information and make decisions in accordance with established policies, procedures, and guidelines Detail oriented with strong time management, organizational, written and verbal communication skills Skilled in non-violent crisis intervention Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs Able to work as a team member and participate within a multidisciplinary treatment team Able to obtain and maintain CPR/1st Aid certification Current, valid California Driver's License with an acceptable driving record and reliable vehicle Minimum 30 WPM typing speed preferred Company Description Agency Mission: The People Concern - OPCC & Lamp Community United - is a community-supported organization in which staff, volunteers, and clients work together with mutual respect to address the effects of poverty, abuse, neglect and discrimination.
